Medical records
Institute of Psychiatry and Neurology in Warsaw based on the Regulation of the Minister of Health
of April 6, 2020 based on Art. 30 section 1 of the Act of November 6, 2008 on
Patient Rights and the Patient Ombudsman
Provides each patient with access to medical documentation regarding his or her health condition.
Medical documentation may be made available by making a copy, transcript or extract, as well as via electronic means of communication, on an IT data carrier - to the patient or his legal representative, or to a person authorized by the patient.
Medical records are made available at the request of the patient, his legal representative or a person authorized by him.
Medical documentation is made available to patients for a fee based on the internal regulation of the Director of the Institute of Psychiatry and Neurology on determining the amount of fees for providing medical documentation.
The fee is not collected when the Patient or legal representative applies for access to medical records for the first time to the extent requested.
The documentation is made available while maintaining its integrity, confidentiality and authenticity, without undue delay.
